What is Peace of Mind?

Peace of Mind (POM) is a specialized program offered exclusively by At Home Senior Care for seniors and their families. POM is designed to best meet the current and future needs of seniors living independently. We understand that you want your loved one to remain independent while being safe and cared for and, most importantly, live their life to the fullest, thriving in their home for as long as possible!


Our POM monthly membership packages will enhance your senior loved one's quality of life while giving you the gift of peace of mind.

 

How does it work?

  • A trusted CarePro will provide up to four flexible visits per month to assist with a variety of quality-of-life activities such as engaged companionship, calendar management and daily structure, transportation when driving becomes a challenge, and to provide purpose and connection when family isn’t available.

  • Each POM member will receive a quarterly activity toolbox filled with engaging activities tailored to your loved one’s interests and abilities.

  • In addition to your CarePro visits, our CarePro Team will check in weekly by video or phone to say hello and see how your loved one is doing. The CarePro will make plans with your loved one for upcoming visits or identify unmet needs and provide reassurance when needed.

  • Evaluating your loved one’s ability to age gracefully in their home, including reviewing wellness goals on a quarterly basis, identifying subtle changes in their independence and making recommendations and/or arranging for next steps or care options, and offering families caregiving resources to assist in understanding how to define their role.

  • The Family Room serves as an online hub for everyone involved in the care of your loved one so you can keep up with daily activities and real-time notes made by CarePros involved in your loved one’s care.
